General Terms and Conditions:

  1. Any deviations from these general terms and conditions are only valid if they have been expressly agreed in advance in writing.
  2. The sessions will take place at the practice address Nijverheidsweg 3-B, 4854 MT. Bavel.
  3. Incidentally, sessions are recorded on video for the therapist's reflection purposes. This is done only with the written consent of the client.

 

Professional professionalism

  1. The professional therapist is a member of the Dutch Association for Drama Therapy (NVDT). This association is affiliated with the Federation for Professional Therapeutic Professions (FVB). Information about this professional association and about the FVB can be found at www.nvdt.nl and at www.fvb.vaktherapie.nl. For clients and referrers: www.vaktherapie
  2. The professional therapist acts in accordance with the guidelines as drawn up in the professional code of the FVB (the professional code can be found under 'quality').
  3. In case of complaints, the first advice is to first discuss your dissatisfaction with the therapist and see if you can resolve it together. If that is not possible, you can rely on the independent complaints procedure of the Federation for Professional Therapeutic Professions. This can be found on the FVB website and is available for inspection in the waiting room of the practice.
  4. The professional therapist is also affiliated with the Dutch Healthcare Association (NFG) and the umbrella organization RBC. For more information see www.de-nfg.nl en www.rbcz.nl

 

Dossier information

  1. The client can receive treatment from the therapist without a referral from a doctor or specialist.
  2. If the  therapist is in the opinion that a different type of help is required for the client, the therapist refers the client, in consultation, to or back to the general practitioner.
  3. The professional therapist is legally obliged to keep file records. This is also necessary to be able to monitor the progress of the therapy process and this is legally imposed by the WGBO.
  4. Your file contains notes about your state of health and information about the examinations and treatments carried out.
  5. The file also contains information that is necessary for your treatment and that I have requested, after explicit permission, from another healthcare provider, for example from the general practitioner.
  6. Before the initial meeting, you will receive an questionnaire in which you will be asked to provide the necessary information. You will also receive a privacy document that you will sign at the start of the therapy.
  7. The main rule for keeping files is in the WGBO. This is 20 years from the date of the recording of each individual item. The term may be longer if this is necessary in view of the treatment (eg if someone has a chronic illness).
  8. The therapist has a duty of confidentiality and will handle your file carefully and confidentially. It is ensured that unauthorized persons do not have access to your information.
  9. Information will only be requested and/or provided to other care providers/data subjects with your permission. You can indicate on questionaire what you will fill in and sign before or during the intake, whether or not you agree with this.
  10. The file includes the following: basic data (name, address, telephone, e-mail, insurance policy number), intake form, treatment contact, treatment plan, reporting sessions, evaluations and results.
  11. For the initial meeting, you fill in a questionaire  in which the above information and personal background and request for help are requested.
  12. The client can view his/her file but not take it with them.
  13. The client submits a written request for inspection to the  therapist. The therapist has one week to update the file. The WGBO law states that it is possible to have your file removed after you have completed the treatment.
  14. The therapist may ask you to participate in a study by Vakeffect, www.vakeffect.nl, in the context of drama therapy. You will receive the necessary information from the  therapist, 
  15.  The data from your file can also be used for the following purposes: To inform other healthcare providers, for example when the therapy has been completed or in the event of a referral to another practitioner. This only happens with your explicit permission To use for observation, in my absence
  16. For anonymized use during peer review (intervision/supervision) A small part of the data from your file is used for financial administration.

 

 Treatment:

  1. In the initial phase, a (provisional) treatment objective is formulated and treatment agreements are made.
  2. After approximately 3 to 5 treatment appointments you will receive a treatment plan that you will sign after approval. Your own goals and wishes are the guideline for the therapy.
  3. Evaluation takes place regularly (every 5 sessions or otherwise if mutually agreed) whether the therapy is helping you to achieve the set goals.
  4. The sessions last 45 to 60 minutes at a time. This will be agreed with you at the start of therapy.
  5. There are regular consultations within the sessions in which the therapist gives an oral report on your developments
  6. There will be no replacement when the therapist is ill. The therapy will then not continue and will be moved to another date/time. In the event of a long-term illness, a replacement will be arranged in consultation with you, or the therapy will be discontinued.

 

Law for the protection of personal information:

  1. Matters such as the right to information, access in the context of privacy and permission for treatment are laid down in the Medical Treatment Agreement Act. https://www.dwangindezorg.nl/rechten/wetten/wgbo
  2. As of May 2018, the General Data Protection Regulation (GDPR) law has been amended and tightened.
  3. The health care invoice you receive contains the information requested by the health insurer, so that you can declare by to your health care insurer: Your name, address and place of residence, your date of birth, Your policy number, The date of the treatment, A short description of the treatment and code,  the costs of a treatment consult.

 

16 year old:

  1. A minor who has reached the age of 16 is capable of entering into a treatment agreement for himself, as well as performing legal acts that are directly related to the agreement.
  2. The minor is liable for the obligations arising there from, without prejudice to the obligation of his or her parents to provide for the costs of care and upbringing.

 

Costs and payment terms:

  1. The costs per therapy session are €75 for a 45-minute treatment consultation and €90 for a 60-minute consultation.

  4. The practice is affiliated with the FVB and the NFG. You must inform yourself in advance about your reimbursement options.

  5. You will receive an invoice by email after each session. You can use this to declare the costs by your health insurer.

  6. Payments must be made within 14 days, stating the invoice number.

  7. In the event of late payment, statutory interest and administration costs may be charged.

  8. If payment is not made, a collection agency will be engaged. Additional costs will be passed on to you.In the event of payment arrears, the therapist is - unless the treatment opposes this - to suspend further treatment until the client has fulfilled the payment obligation. The rates can be adjusted annually.

 

Cancellation of therapy:

  1. It is of course possible that you become ill or that an appointment made cannot be kept for another reason.
  2. You must inform us at least 24 hours in advance.
  3. In case of late cancellation, 100% of the costs will be charged.
  4. The client must be present on time. If a client is late, this will be deducted from his or her session time.
  5. The therapist reserves the right, in case of force majeure, to change the date, time or location of the therapy.

 

Damage and complaints procedure:

  1. The therapist is not liable for material or immaterial damage suffered by the clients as a result of visiting the practice or treatment. This also includes lost items or materials from the client
  2. In the event of damage to the space or materials of the therapist, caused by the client, the therapist will recover the costs from the client.
  3. In case of complaints, the first advice is to first discuss your dissatisfaction with the therapist and see if you can resolve it together. If that is not possible, you can rely on the independent complaints procedure.
  4. The therapist has arranged a facility in accordance with the Wkkggz, she is registered with the NIBIG. The  therapist has taken out collective professional liability insurance for professional therapists with the VvAA.
  5. The therapist has taken out collective legal assistance insurance with the NFG. If a data breach has occurred, I will report it, as legally indicated, to the Dutch Data Protection Authority if this leads to serious adverse consequences for the protection of personal data, or if there is a significant chance that this will happen. I will inform the client if the data breach is likely to have adverse consequences for privacy.

 

Completion of therapy

  1. The therapy will be concluded with a final meeting. You must be present for this.
  2. At the request of the client or his parents/carers and for a fee,
  3. a final report can be written upon completion of the therapy.

Cancellation:

  1. Either party may terminate the agreement at any time.
  2. The client is then obliged to pay the invoices for work performed up to that point.
